Fantastic SIR. Every time when YOU say doing excellent, the positive vibes spreads worldwide. My special THANKS for RUDDLE SIR & Bob Sharp Sir for the extraordinary invention of ENDOACTIVATOR (EA) which is almost like hEArt bridging the shaping & obturation quality. From 2015 Jan I am a user of the same & I classify my practice as before & after EA era as not even 1 case I needed further attention of same treatment again, this itself is a common man's common sense evidence. The added advantage is for CaOH application & removal, MTA adaptation, cleaning of GP in retreatment, sealer application etc made my job more easier. This format reminds me TEST match in cricket with 5 days play, TRS episodes like One day matches of 8-9 hours of play in a full day & Ruddle on Board or Just in time videos like T20 match of 4 hours format in evening time after routines. As Disinfection is the main objective of a RCT, EA is already with endless success , but SLP EA 2.0 is amazing and i am waiting to feel the better experience clinically, the SEM picture of Pileggi almost looking like a heart which reminds us the importance of 3D Disinfection transformed into a 4D. A great philosophical lesson i learnt is, when we choose a field of interest its our responsibility to deliver of our maximum potential, but when The Mother Nature decides to select us to perform a particular department, the Almighty knows how to extract the best from us to serve, help everyone. I think that's what happened in special guest and SIR. I just got around to watching this episode of The Ruddle Show (better late than never) and was so pleased to see the picture of you and Phyllis with Dr. Ingle at the CAE meeting in Winnipeg, Canada in 2016. As CAE president that year, it was my task to organize the meeting and I was delighted that Dr. Ingle could join us. Like you, I learned endodontics from his classic textbook as a dental student. It was a thrill to meet him in person and I was happy to award him honorary membership in the CAE. He was such a lovely gentleman and a giant of our specialty.

In ENDO the most common difficulty is END - For - O, ie., calcified canal, finding the orifice. But the important lesson is keep yourself Calm & Composed, Armamentarium & application of skills, Chelator, great Intention & pre-Enlargement & Discovery of Mysterious original path to apex to reach the peak of satisfaction we can get at end of the performance. Dear Dr. Ruddle:

I write in response to a recent publication of a video in which you are shown comparing Fotona’s LIGHTWALKER dental laser to Edge Endo’s EdgePRO dental laser. See Laser Disinfection & Obturation - The Lightwalker vs. EdgePRO Lasers and Q&A (theruddleshow.com).

It appears that your comparison, which provides negative comments about EdgeEPRO, is based on a single biased Fotona study in which EdgePRO’s directions for use were not followed.
Note, the main technique taught for EdgePRO includes inserting the tip mid-root and always in the presence of a fluid, not 2mm from the apex and dry as you stated in your video presentation. In addition, there are numerous studies that demonstrate that the EdgePRO technique and technology is better at cleaning and disinfecting the canal than the LIGHTWALKER technology and technique.

Critically, I feel your identification of EdgePRO as “potentially dangerous” is a false statement. I am aware of no reported “dangerous” events associated with EdgePRO since its inception. Your identification of LIGHTWALKER as “very safe” and EdgePRO as “potentially dangerous” is improper and I request that you immediately revise your video publication to remove reference to the same and refrain from making such assertions in the future.

I recommend that you review all of the available research and demo the advanced disinfection techniques such as EdgePRO, LIGHTWALKER, and GENTLEWAVE to see which works best in your practice.

In order to clear up any misunderstandings, if you would like, we can come to your practice to go over the research and we can perform demos on extracted teeth. I am confident that you will find EdgePRO to be safe and effective.

I am delighted you are watching The Ruddle Show. Yet, setting that aside, your assertion that I made negative comments regarding the EdgePro laser based on a single published paper is utterly false! On the contrary, my comments were evidence-driven and based on what I have learned about lasers over the past 25 years from peer-reviewed published papers, the textbook entitled “Lasers in Endodontics”, and the opinions of international laser users.

The paper you refer to entitled “Comparison of Er:YAG and Er,Cr:YSGG used in dentistry” precisely describes the technical distinctions between these two laser technologies. Just because you don’t like the information within this paper does not mean this paper is biased. FYI, there are a considerable number of published papers, many of which can be reviewed on my website (endoruddle.com), demonstrating the efficacy, efficiency, and safety of the Lightwalker laser.

Regarding the directions for use (DFU): I never said use a laser in a “dry canal.” Either you have never watched this Show segment or the information you were told was wrong. Regarding the depth of tip placement --- EdgeEndo’s own website (edgeendo.com) has a link to EdgePro’s homepage (edgeproendo.com), which features a video clearly showing the placement of the distal tip of the laser optical wand down a remarkably straight canal and to within “one (1) mm” of the working length! The textbook “Lasers in Endodontics” describes the necessity, yet risks, of placing a laser tip to within 2 mm of the working length, but definitely not midroot!

It is “potentially dangerous” to place a laser tip 1 mm or 2 mm short of the working length, especially considering the many variables. These variables include, but are not limited to, the length, diameter, and curvature of any given canal, the terminal diameter and taper of the final preparation, and the variable skill sets among clinicians. Importantly, another “potential danger” includes the root end proximity to the sinus, mental foramen, or neurovascular bundle.

It is well-documented that a firing laser tip positioned close to length can generate frontal pressure which, in turn, can lead to NaOCl accidents. Further, especially in more minimally prepared canals, the laser wand itself will tend to displace the miniscule remaining fluid volume and will certainly contact dentin. This phenomenon has been shown to induce melts, thermal burns, and ledges. The EdgePro technique shown on its website instructs placing a laser tip to within 1 mm of working length, then precisely withdrawing this tip at a rate of 1 mm/sec and doing this 3X/canal. In sharp contrast, the Lightwalker method confines the laser wand within the pulp chamber only, and efficiently cleans and disinfects all prepared canals simultaneously.

Chuck, the good news is, this point/counterpoint discussion will spark a conversation regarding lasers. I guess you heard me say, repeatedly, that both lasers have efficacy, both lasers clean very well, and EdgePro’s price point provides a more affordable disinfection method in the marketplace.

I am a very slow clinician; I had a hard time using Resin Sealers with Cold Lateral Compaction technique. But the reason why I thought a resin sealer would be suitable for CLC is the inherent deficiency of a CLC just because of the fact that it is cold! I thought the relative toxicity of a resin sealer would make up for the lack of hydraulics in a cold obturation technique. But hopefully, I will start using Kerr Pulp Canal Sealer EWT once I implement a warm obturation technique. .

Such a legend! I was blessed to have Dr. Ruddle make me hold an Endomotor for the first time in my life at the APEC conference, Delhi. One of the most humble and inspiring teachers I have ever met, who never seemed to let go of Phyllis's hand at those endo conferences 😅 (they're honestly too adorable together).

I have been following the advanced endo education on his website since my first year of postgraduate study. It helped me understand fundamentals of endo much quicker than any of my batchmates just because of his fabulous teaching which inspired me to implement those tips and tricks into my clinical practice.

I recently heard of ruddle show.com on Howard Farran's podcast and I haven't stopped binge watching through all this new content - not just for the educative content, but because it feels like I know him better through his family and the endo community talks. I have never been so driven! Especially during these tough times.

Can't wait to meet Dr. Ruddle and Phyllis in person at WEC 2020NE, India. Will stay tuned for his online session on retreatment which is scheduled next month.
